    Prevalence of rosacea in the general population of Germany and Russia - The RISE study.

     

    Abstract

    BACKGROUND:

    There is an unmet need for general population-based epidemiological data on rosacea based on contemporary diagnostic criteria and validated population survey methodology.

    OBJECTIVE:

    To evaluate the prevalence of rosacea in the general population of Germany and Russia.

    METHODS:

    General population screening was conducted in 9-10 cities per country to ensure adequate geographic representation. In Part I of this two-phase study, screening of a representative sample of the general population (every fifth person or every fifth door using a fixed-step procedure on a random route sample) was expedited with use of a questionnaire and algorithm based on current diagnostic criteria for rosacea. Of the subjects that screened positive in the initial phase, a randomly selected sample (every third subject) t`hen underwent diagnostic confirmation by a dermatologist in Part II.

    RESULTS:

    A total of 3052 and 3013 subjects (aged 18-65 years) were screened in Germany and Russia respectively. Rosacea prevalence was 12.3% [95%CI, 10.2-14.4] in Germany and 5.0% [95%CI, 2.8-7.2] in Russia. The profile of subjects with rosacea (75% women; mean age of 40 years; mainly skin phototype II or III, majority of subjects with sensitive facial skin) and subtype distribution were similar. Overall, 18% of subjects diagnosed with rosacea were aged 18-30 years. Over 80% were not previously diagnosed. Within the previous year, 47.5% of subjects had received no rosacea care and 23.7% had received topical and/or systemic drugs. Over one-third (35% Germany, 43% Russia) of rosacea subjects reported a moderate to severe adverse impact on quality of life.

    CONCLUSION:

    Rosacea is highly prevalent in Germany (12.3%) and Russia (5.0%). The demographic profile of rosacea subjects was similar between countries and the majority were previously undiagnosed.

In a recent study from our department 7/54 adult atopic dermatitis patients treated with systemic immunosuppressants were referred by ophthalmologists because of severe ocular involvement keratoconjunctivitis with or without keratoconus associated with milder skin symptoms (2). We report the case of a woman affected by atopic dermatitis and asthma in childhood who developed at 63 years of age conjunctivitis associated with blepharitis without causative contact allergen identification at skin testing. She received multiple topical and systemic treatments including cyclosporine 2mg/kg/day stopped after 4 days because of digestive adverse effects.

"Anecdotal results of reduced erythema and acne with the use of OnabotulinumtoxinA (Botox®, Allergan, Irvine, CA, USA) for cosmetic purposes led Dayan et al to experiment further in 13 patients with rosacea. Over the course of 2 years, patients received intralesional microdroplet injections (0.05 mL) of onabotulinumtoxinA in a dilution of 7 mL of saline solution per 100 units. The results indicated significant reduction in erythema and flushing of the affected area 2–4 weeks after treatment. Their predecessors reported similar results after a 26-year-old male was injected with 10 units of onabotulinumtoxinA at 1 cm intervals into one cheek."

    "Finally, a formulation of low molecular weight hyaluronic acid sodium salt 0.2% cream (Bionect® Cream, Innocutis Holdings, Charleston, SC, USA) applied twice daily for 8 weeks showed a statistically significant reduction in papules, erythema, burning, stinging, and dryness in a small study. Along with excellent tolerability, a reduction in papules, erythema, burning/stinging, and dryness was clearly apparent by week 4 and remained by the final visit at week 8. Notably, erythema was reduced by the greatest amount, with an approximate 50% improvement by week 2. This study demonstrates the importance of skin barrier maintenance and repair in the pathogenesis of rosacea. With a hydrophilic structure, low molecular weight hyaluronic acid penetrates the stratum corneum where it interacts with fibrin and collagen to support remodeling of the extracellular matrix. This mechanism, along with induction of the antimicrobial peptide β-defensin 2 release from keratinocytes, ultimately accelerates re-epithelialization and reduces pain and edema."

    "In another single-center, open-label study, a group of 30 patients with rosacea of variable severity were treated with 4% Quassia amara extract topical gel for 6 weeks. Reportedly, Q. amara possesses antiparasitic and anti-inflammatory properties that have the capability to decrease the inflammatory response with few complications. Efficacy comparable with that of first-line topical treatments like azelaic acid and metronidazole was observed after Q. amara treatment, with superb tolerability."

  8. "Long story short, a dermatologist diagnosed with rosacea. Which doesn’t have a cure. Oh yeah and is treated “simply” with 2 doses of an antibiotic daily, and 2 applications of a medicated cream daily. Sigh....But I was desperate to start looking like myself again. So I looked into this “It.” Turns out it is sold in Ulta, and, after viewing their videos, my God, their products really do fight the good fight against rosacea, meaning it covers up the damn redness. (Sorry for doubting you, Ma)."

    Dave: "That being said, in January I went on a weeklong trip to the Virgin Islands. During that week I snorkeled several times, probably for 2 hours total. I was convinced beforehand that the saltwater would irritate my skin (anything irritates my skin). I couldn’t have been more wrong, my skin looked better than it had for 3 months using all types of medicine from the dermo."

    Lynn and husband: "Both my husband and I also have Rosacea. We too have noticed when we return from a trips from Florida or Hawaii, the Rosacea has cleared up."

When I got home it went right back to being red and irritated. Is it just because I got some sun and it looked more tan? My face did not get irritated by the sun, it actually felt way better."

    BVokey posts: "The exact same thing happened to me while in Hawaii recently and also happens whenever I am staying in a warm climate. I was there for two weeks and my Rosacea cleared up almost completely however slowly returned once I got back home to the cold weather and back to the grind."

I went to visit my girlfriends family in Canada on the way and my skin was so bad whilst I was there that I said to her "I don't think I am going to last 2 weeks in Central America," I was dreading it. I got to C, America and after 3 days my skin started to improve. I was in the sea surfing, my skin started to burn less, sensitivity went down, even had a few drinks that I hadn't had for years without a flare up. Whether its a combination of stress, humidity, surf, sea, sun, I don't know, but it showed me that it CAN get better. It might not always seem the case, much like the period that I am going through now, I need to remind myself, last year I went to C, America in 30C heat and it was manageable."

    Seb91 [post no 34] didn't mention the South Pacific or a beach but did say this: "I went to Mexico for 4 months with my girlfriend 2 years ago, a trip I was absolutely dreading because my skin was in a bad place, and I ended up being absolutely fine and having the best time, without much pain, sensitivity, and redness. Not sure why? Maybe it was because I was doing something I love (surfing), humidity, the air quality, I really don't know. But seeing my skin on that trip said to me that this isn't the result of lasers, this is a result of an environmental change."

    BVokey [post no 1] reports, "Honorable mention: going to the beach and swimming in the ocean, warm/hot showers, hot tubs, etc all actually reduce my redness for a short time. I think it's related to my entire body getting water. The biggest healer of the above is swimming in the cool ocean saltwater - it drastically reduces my redness to an almost pale pink. Being out in the sun as much as possible as long as I am wearing sunblock,,,"

    I use the Gillette Fusion Razor and found that electric razors were more rough on my face causing redness. So you will have to figure out what is best for you.
